* Genset had 250 + 500 hrs service completed @ 8600hrs on 2/4/22
* yellow flashing light was faulty and was replaced 2/4/22
* Tag line attached to fwd spring at CP1 western berth as there was no tag line 31/3/22
* fender at home berth changed out and socks replaced on fender on bow behind Genset box
* gas detector due for service 20/4/22 this was serviced/calibrated on 6/4/22 , There was no old certificate in section 12 of SMS and am unsure if we are to be issued with one ?
* whilst berthing at CP3 on the scheduled 0830 run 4/4/22, the vessel was alongside just taking up the last 300 mm on the fwd spring when the blue arrow jet control indicated that the stbd bucket was stuck up. After checking the controls with all lines secured after the passenger had disembarked it was found that the stbd bucket was indeed stuck in the fwd position and not responding to command of astern. The throttle was still functioning and the bucket remained stuck in the fwd/ahead position. The machinery and buckets were inspected and no fault apparent. I reset the controls and haven’t been able to fault since after testing emergency controls , mouse and lever controls numerous times As a precautionary measure I have also backflushed both jet units, we will continue to monitor and report any deficiencies as they arise, also of note I had 20 mins prior operated the vessel in the full astern position whilst conducting an emergency stop drill prior to berthing at CP1 @ 0820 hrs , which may have possibly had something to do with the fault?
Luke Holzberger also had the same fault with the bucket stuck up on 17/4/22, Brody has attended the vessel at 0800 hrs on 17/4/22 at CP1 and calibrated JIM 2 jet REV stuck fault and calibrated throttles , all good after calibration so far
19/04 incident occurred at cp3 involving jet bucket stuck in fwd position while berthing, causing vessel to ground on rock wall, maintenance attended and replaced a sensor on the bucket actuator and recalibrated unit. Brodie will follow up in near future to replace 3 other sensors. ConocoPhillips marine specialist informed crew Wednesday morning that they will be conducting an inspection on Thursday in relation to the incident.
* To clean the Genset strainer in the port void, both crew are required to have confined space training. The entry to confined space is only to occur when vessel is berthed in marina or at slipway. Entry to confined space is not to be conducted when the vessel is onsite at client facilities, I have been advised by Willie Ewins if the crew aren’t able to do this task we are to request maintenance to complete task.
* Chart corrections up to date @ 14/4/22 edition 8 / 2022
* Test and tag electrical was completed 12/4/22 now due in July
* insurance certificates have expired 31/3/22 , office notified and waiting for new certificates to be provided to vessel, please update vessel certificate register on dataweb when new documents arrive
* Port main T piece for exhaust mixing cooling water that was leaking was removed and welded up 12/4/22 no leaks apparent
* port exhaust lagging was repaired 12/4/22 and tail of clamp removed
* 250 hr service for mains completed 12/4/22 eng hours @ 1282 which was anodes changed out only
* pitting and corrosion at chine at the rear of both engine rooms have had 2 x coats of penetrol applied to help slow down corrosion
* MRF submitted 9/4/22 for stbd main engine coolant expansion tank cap drain # 7481 and was rejected , have zippy tied guts of drain for maintenance to find locally at expansion tank, MRF # 7485 submitted for another bung in stbd exhaust manifold leaking coolant, the coolant is coming out of the cap also due to being overfilled..... and coming down the side of engine over filter and onto beam, maintenance state to keep coolant levels below max..... halfway on sight glass is more than sufficient, you can watch the coolant level go up and down on the sightglass with cctv as it expands as it it heats up and then and cools. I have placed a stick with a max. mark clearly labelled in the stbd eng room underneath the hatch, wedged at the step that enters the aft end of the engine room, to be used for coolant level checks as the sight glass is unreadable.... the port side coolant level is clearly visible through the sightglass. If the coolant is inadvertently topped above this max. mark, there is a siphon hose ( I did check the dictionary for the spelling of siphon ) in the bund tray with the coolant drum to remove excess coolant from the expansion tank.
* new black rubber mat at boarding area, gangway seems to slide easier on this new mat, crew to monitor gangway when in position for boarding to ensure it remains in correct position when vessel is alongside and adjust as required
* stbd gearbox has noticeable clunk when clutching in, maintenance have attended vessel 0700hrs 10/4/21 and deemed serviceable and to continue operations for now
* electronic chart updates have been done by Modern Marine Electronics 14/4/22, have added in notes section in IPAD for reference as well as certificate register on dataweb
* 15/4/22 new msds sheets added to msds register for all fire extinguishers onboard
* 15/4/22 port engine idling at 650 rpm and jumps to 700 rpm when clutching in. Throttle setting reads 4% at idle when clutched in on normal systems mouse or lever controls. Emergency controls throttle reads 5% when clutched in. The stbd engine remains at 0% throttle when idling and also 0% throttle when idling clutched in @ 650 rpm. Swapped over throttle relays and no change. Spoke with Brody and he requested we monitor and notify if changes again
* 14/4/22 + 16/4/22 + 18/4/22 fuel nozzle at service wharf is leaking badly, there is only 1 to choose from , extra oil spill pads required whilst refueling, operations manager notified and information tag has been attached 0800 hrs 14/4/22. There have been no fuel receipts available for the whole swing and it has just started printing out again on 14/4/22
* the port aft door curtain that is usually secured in the up position has one of the retaining straps come adrift, I have ordered 316 stainless steel press studs x 4 to repair but we haven’t received them yet, it is temporarily secured with zippy ties, the retaining strap is located in the 5 lt white bucket under the port dash when the press studs turn up to be reattached
* l have ordered an assortment of out of service / information / caution / danger / Do not operate tags, with stores order submitted 18/4/22 , Luke has sourced some caution tags from APLNG for the interim, located in the defib tray
* sanitary bin was changed out 16/4/22
* nil fluids added for whole swing , just topped up port main engine oil with 1 lt and topped up port hydraulics on jets with 200 ml 16/4/22
* degreased and cleaned both engine rooms 18/4/22
* fuel booking for Tuesday 19/4/22 0800-0830 inside western end service wharf
* my last day of swing is on Spirit 19/4/22 so I hope all is in order when you join vessel
