Joined vessel 24/5/23
25/5/23
* Vessel departed CP1 for scheduled 0700 run with 5 x pax onboard and whilst bringing the vessel up to cruise speed the analogue guages increased the rpm of the stbd main engine to 3000 rpm when engine should be reading 1750 rpm and the other guages also increased and engine management system displaying check engine and then shut down engine warning lights, The stbd engine was shutdown and the gauges continued to display erratic readings going from low to high without the engine running, only ignition key on. Engine was restarted and diagnosed to be responding correctly to throttle control settings however low voltage alarms and low coolant alarms were now coming up, engine room inspected and nothing out the ordinary. The stbd engine was shutdown 3 times reset ignition and restarted and then after the fourth time the alarm panels and warnings were all back to normal and the vessel was continued to operate for the am ferry services without the fault occurring for the rest of am operations , maintenance advised of issue amd duty operations request incident report to be submitted, video of guages with erratic readings forwarded to management , maintenance attended and checked out diagnostics
* Maintenance replaced the signs regarding hatch to be closed at all times on the foredeck port and starboard
26/5/23 Torch that was supplied to vessel was unsuitable and Willie has given us led lenser torch and old torch returned to office 👍
30/5/23 * Alarms on stbd main at startup am, indicating critical low coolant in engine (Fault 235 FM1 1 PID 111 )and diagnostics to stop engine as well as low voltage alarm, Brodie attended vessel after am runs and replaced coolant sensor , crew to monitor and advise
31/5/23 * No faults apparent on vessel alarms after coolant sensor was changed out with second hand one, it was agreed that the stbd main needs a coolant expansion bottle and Brodie was intending on ordering one for the vessel 😃
6/6/23 * Returned to vessel after 3 days of serving on other vessels and starboard Simrad failed to boot up, it has power to the unit but won't boot up, I found an MRF # 8728 submitted for it stating freezing up then shutting down? I have removed the chart chip from the stbd unit and installed in the port Simrad so now we have a chart available using split screen with radar + chart however the vessel has no echo sounder available with the stbd Simrad unit u/s
* Also found MRF # 8729 that was submitted 5/6/23 for pilot chair stating it comes off its mount when leaning back requesting to improve locking mechanism which was attended to by maintenance 6/6/23 and MRF closed out 8/6/23
